Applications
Basic Criteria
The ideal client meets the following basic criteria:
- Spinal cord injury at any level from C-2, but MUST be able to breathe on their own
- Complete or incomplete diagnosis
- In general good health
- Cleared by a physician to start an intensive physical exercise program that includes load-bearing exercises
- Meets weight for height parameters
- Highly motivated with a positive attitude
- Believes in being able to walk again
Prior to participating in our program, all clients over six (6) months post injury (permanent or visiting) must provide a recent bone density scan, along with an interpretation of the results summarized by a doctor. Project Walk does not read or interpret bone scans.
Project Walk reserves the right to reject clients who do not meet entry criteria.
Application Forms
To schedule a visit, please fill out the Application Form below, in its entirety, and email to applications@projectwalk.org or fax to (760) 431-1598. We will notify you by email or phone when we receive it, so be sure to include your email address. This form must be completed and submitted to Project Walk prior to scheduling your visit.
The Application Form consists of eleven (11) pages with five different sections:
Client Application, Service Agreement, Release of Liability, Billing Information
and Skin Check. Make sure to fill out each page in its entirety.
